21 mm is the box and open-end size of the BOERM21, a 12-point metric ratcheting combination wrench from Blue-Point with a 15° offset. Snap-on lists it at 39.4 (A), 43.2 (B), 13.7 (C), 9.4 (D) and 266.7 (E), all in mm. It is the standard-handle ratcheting version of the combination style.
